I’m scheduled for a visit

We are looking forward to seeing you at our office. You can make sure that you are prepared for your visit by following these helpful guidelines.

New Patient Consultation

Bring all of your medications and supplements that you are currently taking along with any records pertaining to your care. This includes bloodwork, office visits and any special testing that may have been done by other physicians. Make a list of questions you may have prior to the visit so you can get all of your questions answered during your consultation.

Hospital Follow-up Visit

Bring all of your medications and supplements that you are currently taking, including any new medications that were prescribed when you were discharged from the hospital. In order for the physician to serve you best, be prepared with any questions you may have about findings during your hospital stay, and any symptoms that you may have since your discharge.

Office Follow-up Visit

Follow-up visits should be scheduled before you leave the office to ensure continuity of care. Be sure to let your physician know if you have a change in your physical status which would include new or improved symptoms or a decline in your health.
